If you would like to open an account with us please download our credit application, complete it and fax or e-mail it back to the Accounts Receivable Contact Information that follows. Please note that we can sometimes secure same day approval for applications that are fully completed and signed. Otherwise, we must mail out inquiries to your references and the process generally takes two to three weeks. In any case, please have an authorized person complete and sign the Banking Reference Section, as banks generally demand written authorization prior to releasing credit information. It will delay your account application if you do not complete this section and your bank declines to supply us with the information we need.
You may also download our Credit Card Authorization Form, complete it and fax it back to me and we will be happy to provide you with immediate service.
